University Reporting Form

Do not use this form to report events presenting an immediate threat. Reports submitted through this link may not be reviewed outside of normal business hours or immediately. If there is an immediate risk to health or safety, please call 911 to connect with medical support or local law enforcement. For an accessible version of the Title IX report form, please visit the Accessible Report Form.

To better understand all your reporting options, please refer to I Want to Report an Incident and the FAQs about ReportingBy using this online reporting form, you are submitting information to the University of Illinois Title IX Office and the Office for Access & Equity.

Anonymous Reporting

An individual may report an incident without disclosing his/her name, identifying the respondent, or requesting any action. Please note that choosing to make an anonymous report can significantly limit the ability of the University to respond. This information will be used for statistical purposes as well as for enhancing understanding of our campus climate so that we may strengthen sexual misconduct response and prevention efforts.

Responsible Employees

Responsible employees must immediately report allegations or disclosures of sexual misconduct (per the Sexual Misconduct Policy) to the Title IX Office. Because the definition of responsible employee is so broad, you should consider yourself a responsible employee unless you qualify as a confidential resource. (Confidential resources on campus include the Counseling Center, the Women’s Resources Center, and McKinley Health Center.)

After a Report

The Title IX Office receives all reports of sexual misconduct made to the U of I. When a report is received, the office, or a designee in the Office for Access & Equity – EEO Division (“EEO”) will review the details of the report and assess for any immediate safety concerns and reach out to the person who is alleged to have experienced the harm (the “complainant”) if their identity is known, typically via email, sharing Rights and Options and offering to meet.

Meeting: What to Know

  • The meeting is optional.
  • You can bring a person with you for support.
  • A Title IX Office or EEO staff member will discuss any safety concerns, go over your rights, talk about and facilitate available supportive measures and resources, and review available procedural options (including university and police options) to resolve the concern.
  • You can share as much or as little information as you want regarding what happened.
  • Making a report does not automatically initiate an investigation; complainants can request an investigation (submit a complaint) or make a request for no university action.
  • Supportive measures are available with or without filing a complaint.
  • The University reserves the right to investigate or otherwise address any report, regardless of when it is made, based on concern for the safety or well-being of the University community.
